Intake and Assessment

Intake and assessment consists of an interview and data gathered from a variety of sources, including the individual’s:

  • Individualized Program Plan (IPP)
  • Individualized Education Plan (IEP)
  • Ancillary reports
  • Medical records
  • Family input

Direct Service

An Individual Service Plan (ISP) is created for each consumer outlining individualized instruction, based on assessment data. ISP’s contain measureable goals with quarterly updates to track progress. Common areas of instruction and support include:

  • Home maintenance
  • Medical and well being appointments
  • Medication organization and management
  • Mobility training
  • Benefits management
